Student Accident Plan

The Archdiocese of Baltimore is once again sponsoring a two-tiered student accident insurance program.  The plan includes a voluntary basic program and an Archdiocesan paid catastrophic plan. Any student enrolled in an Archdiocesan parish or regional school (including pre-K) which participates in the Archdiocesan insurance program is eligible.  To determine whether or not your child is enrolled in a school that participates in the Archdiocesan insurance program, please refer to this listing.

Enrollment
To enroll your child, please print and complete the application. Once you have completed the application, simply mail the application along with the applicable premium payment to the program administrator:

All applications are to be sent directly to the program administrator identified in the application information. No applications or premium payments are to be submitted to the school.

Filing a Claim
If your child is injured and a claim needs to be filed, complete the claim form. Please remember it is the responsibility of the parent or guardian to  file a claim directly with the claim administrator. Each school's administration will have claim forms available should you be unable to access the form on this website.
